If someone reports they cannot be vaccinated due to a medical, personal belief or religious reason, they must fill out this form as well as provide proof with a negative COVID-19 test with results obtained within 72 hours before the OVATION event. Children who are not yet eligible to receive the vaccine are also required to submit a negative COVID-19 test with results obtained within 72 hours of the event.
