CARLISLE, Ohio —

Ohio State Highway Patrol are investigating a fatal crash that left a bicyclist dead Monday evening near Carlisle.

According to OSHP, just after 6:30 p.m. a man was riding his bicycle along the Franklin-Madison Road when the driver of a passing vehicle tried to get around the cyclist.

However, the cyclist did not notice the driver and instead turned and crashed into the car, police said.

The cyclist died on impact, officials said.

Police identified the victim as a 70-year-old man. No other information has been confirmed at this time.

First responders arrived on scene and attempted to perform CPR, but they were unable to revive the victim, police said.

Franklin-Madison Road was closed briefly during the crash investigation but has since reopened.

DRY RIDGE, Ky. — One person is dead following a crash Friday night in Grant County.

According to Kentucky State Police, troopers responded to a two-vehicle crash around 9:23 p.m. on Hwy. 467 in the 6600 block of Warsaw Road in Grant County.

Investigators said a preliminary investigation revealed a driver of a red Mazada 6 crossed the double yellow line into oncoming traffic and hit a gray and black Ford Focus.

The Grant County Coroner's Office pronounced the driver of the Ford Focus, Harvey Nunn, 46, of Williamstown, dead at the scene.

The crash remains under investigation by Kentucky State Police Post 6 Detectives and Collision Reconstruction Units.

ST. CLAIRE TOWNSHIP, Ohio — A 33-year-old Seven Mile man has been identified by the Butler County Coroner’s Office as the person who died Wednesday when his SUV was hit by steel coil that fell from a semi-tractor trailer.

Blake A. Mulder, of North Main Street, was killed when his Ford Edge was hit by a steel coil as he drove south on U.S. 127 in St. Clair Twp. He died of multiple traumatic injuries and his death was ruled an accident, according to the coroner’s office.

The crash that happened about 6:20 a.m. on U.S. 127 near Morganthaler Road, closed the busy road for about eight hours.

A semi-tractor trailer owned by Total Package Express Inc. was traveling north on U.S. 127, also called Hamilton Eaton Road and was carrying a steel coil, according to the sheriff’s office. The steel coil became unsecured, fell into the southbound lane and struck a Ford Edge, the sheriff’s office said. The driver of the Ford Edge was pronounced dead at the scene.

The driver of the semi called 911 to report a coil had fallen in the road, but he didn’t know another vehicle had been hit, according to the call.

“I am on 127 north of Hamilton. I am in a big truck. I haul steel and one of my coils came off my trailer,” the truck driver told dispatchers. He said he was about an eighth of a mile from where the coil fell.

When asked where the coil went, he said “traffic is stopped back there so it must be in the road. Nobody is injured or anything, but it came out of my trailer.”

The truck driver said another motorist driving a truck had run him off the road into the grass several miles back.

The truck driver said he had stopped the other driver to talk about the incident, but he didn’t seem to understand what he was saying.

“I came across the railroad tracks and around the corner, and I just heard something,” he said. Then he stopped and discovered he was missing a coil.

On the other side of the crash, a woman called 911 reporting a SUV in a ditch with a strong smell of gasoline.

“There has been a bad accident, there is debris all the road,” the woman told dispatchers. She and others could not see in the vehicle to see if anyone was inside.

She said she could only see one vehicle. “I don’t even know what they hit.”

CINCINNATI — A woman was killed and five people were hospitalized after a head-on crash along Winton Road on Sunday evening, according to Cincinnati police.

An initial investigation found that 30-year-old Brittany Green had been driving north on Winton Road in her 2005 Chevy Trailblazer when she lost control of the vehicle and struck a 2011 GMC Acadia in the opposite lane, police said.

Green was rushed to University of Cincinnati Medical Center in serious condition and later died of her injuries. A passenger in her vehicle was also hospitalized with serious injuries.

The driver of the Acadia and three passengers, including two juveniles, were also treated for non-life-threatening injuries at area hospitals.

Police have not yet determined if speed or impairment were factors in the crash.

BROOKSVILLE, Ky. — Two people are dead after a crash Wednesday afternoon on the AA Highway in Bracken County, the coroner said Thursday.

According to Bracken County Coroner John Parker, the crash involved two vehicles, each carrying one occupant. One driver — whom Parker identified as Joshua Martin of Lake Waynoka, Ohio — died at the scene of the crash around 4 p.m. Wednesday near Hilton Lane.

The other driver, whom the coroner did not immediately identify, sustained injuries and was transported to the University of Cincinnati Medical Center, where they later died.

Officers with the Bracken County Sheriff's Department and Kentucky State Police responded to the crash and were investigating.

TATE TOWNSHIP, Ohio — Two people, including a 17-year-old, were killed and two others were seriously injured after a crash in Tate Township Friday afternoon, according to the Ohio State Highway Patrol.

Police said the driver of a 2005 Honda Accord traveling east on Bethel New Richmond Road lost control and struck a 2015 black Ford Escape in the opposite lane. The Accord then traveled off the road and struck a utility pole. The force of both impacts split the vehicle in half, police said.

A 17-year-old passenger in the Accord, Austin T. Rose, was ejected from the vehicle. Both Rose and 26-year-old Jose F. Noland were pronounced dead at the scene by the Clermont County Coroner. Neither person wore a seatbelt at the time of the crash, police said.

The driver and a 16-year-old passenger in the Escape were transported to area hospitals in serious condition. Both were wearing seatbelts at the time of the crash.

The crash remains under investigation by OSHP's Batavia Post.

MONROE, Ohio — The Monroe Police Department is investigating a fatal two-vehicle crash that occurred just after 3 p.m. Friday.

A tractor-trailer collided with a silver sedan at the intersection of N. Main Street and State Route 63.

The driver of the silver sedan was dead when crews arrived at the scene.

The sedan’s passenger was transported to Miami Valley Hospital via medical helicopter where they succumbed to their injuries.

The driver of the tractor-trailer was not injured.

The names of the victims have not been released.
